Practice Catchment Area

As a guide, the practice covers a radius of approx. 12km (8 miles), including Montrose, Hillside, Dun, Farnell, Lunan, Inverkeilor and Ferryden.

How to Register as a Patient

When you want to register with a new GP practice, you’ll need to follow these steps to ensure that your new GP practice has all your medical history and your medical records.

If this is your first registration with a GP practice in the UK, medical records from outwith the United Kingdom will not be traced.

You can only register with a GP practice if you are staying in the area for more than 3 months. If you are not, you may still be treated, but as a Temporary Resident.

You will need to complete the General Practice Registration Form (GPR) and a New Patient Health and Lifestyle Questionnaire. You can also register for online patient services to order repeat prescriptions. All forms can be downloaded and printed from the Forms to be Completed section below:

Forms to be Completed

  • General Practice Registration Form: To be completed in full. We request proof of identity for all new patients registering. This can be in the form of a photo driving licence, passport, bus pass etc. Parent’s identification is adequate when registering children aged under 16.
  • New Patient Questionnaire: We require information which will be held securely in your medical record. Please provide as much information as you can.
  • Online Services Registration Form: Register for online services to order repeat prescriptions and book certain appointments online.

Until you are fully registered with the practice, you will remain registered with your current GP Practice.

Until we receive confirmation of your past medical history and medication we will not be able to issue any repeat prescriptions. You should ask your previous GP for a supply until your registration process is completed.

It is important that you complete and return all forms including your identification (ID).

You can post your completed forms and ID in the letterbox at the end of the heath centre building, or alternatively email everything to [email protected].

New Patient Registration

Once registered you will receive an SMS text message with the new patient registration questionnaire to complete if you have provided us with a mobile number. Otherwise, all new patients are required to attend new patient registration with the practice nurse or doctor before registration can be confirmed.
